You must receive a return authorization (RA) before sending back any product for any reason. Call customer service at 1-888-776-3322 from 9:00am to 6:00pm Central Time Monday - Friday.

SportStar Chinstrap limited warranty

SportStar chinstraps are warrantied against defects in material and workmanship for a period one year from the date of purchase. Proof of purchase is required. Misuse, altering this product, or cleaning with any solution other than soap and water will void the warranty. SportStar at its own discretion will either repair or replace defective products. Customers should call SportStar directly at 1-888-776-3322 to receive a return authorization.

SportStar Mouthguard LIMITED WARRANTY

SportStar Athletics, Inc. warrants this mouthguard to be free from material and manufacturing defects. SportStar Athletics, Inc. warrants that this mouthguard will protect the wearer from injury to dentally sound natural teeth of the original purchaser when properly fitted and worn according to this instruction sheet. This mouthguard should not be worn with braces, loose teeth, loose caps, or bridges. This warranty extends only to a supervised game or practice session under the direction of a trained coach. This warranty is valid for one (1) year from the date of original retail purchase. SportStar Athletics, Inc's. obligations under this warranty are limited to the lesser of: The actual dental expense incurred by the original purchaser; or $625.00 per injured or replaced tooth, up to a maximum of liability of $20,000 per incident. SportStar Athletics, Inc.'s financial responsibilities shall be reduced by the amount of any insurance payable or collected for the injury claimed. This warranty does not cover orthodontic treatments or preparation of a tooth to be used as an anchor for a bridge. The warranty shall be in effect only if the claimant followed all fitting and wearing instructions accompanying this product.

Under this warranty all claims must be filed within 30 days after the occurrence of the injury. SportStar must receive the following confirming documentation via certified US Postal Mail:

  1. A notarized statement and game film if available from a participating game or practice official, supervising coach, school superintendent that the mouthguard was in proper use at the time of injury; and
  2. An original sales receipt for the retail purchase and a copy of all dental records and x-rays (required) which verifies the injury and accompanying documents by a licensed dentist providing necessary dental treatment within thirty (30) days after the date of injury; and
  3. Copies detailing any benefits from dental or medical insurance carriers and all submitted dental and medical insurance claim forms related to the injured tooth (teeth) and all dental work that was performed as a result of this injury and;
  4. The mouthguard must be returned to SportStar Athletics, Inc. with all the above documentation. All documents, materials, and the mouthguard become the property of SportStar Athletics, Inc. upon receipt. SportStar's liability is limited as specified above. SportStar Athletics, Inc. disclaims all consequential damages and liability for loss, injury or damage based upon negligent design or defects in materials and workmanship for the enclosed mouthguard. Keep this dental warranty, disclaimer and a copy of the store receipt for your records. This limited warranty is governed by Texas law.

In case of injury, mail mouthguard, original store receipt, the notice of injury, and accompanying documents to SportStar Athletics, Inc. at the address below. If you do not accept all these terms, please send the un-used mouthguard back to SportStar with the original receipt for a complete refund.
